READ VERY CAREFULLY THE MATERIAL BELOW PRIOR TO REGISTERING FOR THE 2011-12 YEAR...
To register once you have been notified that your child has been selected to join a 2011-12 FC Westchester team follow the directions below for that particular team...again, read carefully:
o To register for the U12-U18 Boys FC Westchester Premier Teams CLICK HERE (This is only for premier teams, for Academy and Pre-Academy teams keep scrolling down):
Follow all the instructions via the online system...and go through all the screens all the way to the end.
After you register via the online system, you must mail the following to Carolyn Murphy, FC Westchester Premier Team Registrar, 300 Main Street, Apt. 2C, White Plains, NY 10601. o New to the club players must submit 1) a copy of their birth certificate or passport; 2) a completed medical release form (CLICK HERE TO OBTAIN THIS FORM; 3) two passport-sized photos, with player’s name, "FC Westchester Premier and "Team Name" printed on the back should be sent to our registrar.
o Returning players must also go online and register and also supply two headshot pictures to the registrar.
o Returning and new to the club players, in registering online, also need to make the first installment of the club dues via credit card when you register (or you can also submit a check made out to FC Westchester) for the first half of the yearly dues ($825 for the U12 team and $900 for the other teams) and mail check to the registrar at the address above.
o For 2000/2001 and 2002-03 Pre-Academy Training Teams:
To register for the 2000/2001-02/03 Pre-Academy Training Programs you can do so by CLICKING HERE. ThIs online registration process is fast and easy; just follow the simple instructions after you get into the online registration system.
After you register via this online system, you must mail the following to Hilary Unger, FC Westchester Pre-Academy Registrar, 214 Waverly Road, Scarsdale, NY, 10583:
1) a copy of their birth certificate or passport; 2) a completed medical release form (CLICK HERE TO OBTAIN THIS FORM); 3) two passport-sized photos, with player’s name, "FC Westchester" and team name printed on the back should be sent to our registrar.
Note: the online registration system for the 2000/2001 and 2002/03s consists of several screens...so keep on clicking continue until you get to the end of the process.
o For the FC Westchester 1999, 1998, U14 Pre-Academy and U15 Pre-Academy Teams Only (Do Not Use Online Registration...see below):
o All 1998, 1998, U14 and U15 Pre-Academy team players need to complete a US Club registration form which can be found by CLICKING HERE.
o All need to send to Hilary Unger, FC Westchester Academy Registrar, 214 Waverly Road, Scarsdale, NY 10583 the following:
1) the completed registration form, 2) two passport sized pictures, and 3) the first half of the registration fee ($900) via a check made out to FC Westchester. New-to-the-club players MUST also 4) mail a copy of their proof of birth (passport or birth certificate).
For new to the club players, the team manager will collect a check for $205 per player for a new uniform kit, which will include a blue jersey, a white jersey, white shorts, blue shorts, white sox, blue sox, a warmup and a bag.
For the U16 and U18 U.S. Soccer Development Academy Teams (Do Not Use Online Registration...see below):
o All players (returning and new to the club) accepted onto these teams will be given/sent a United States Soccer Federation waiver form shortly after being accepted into the Academy program.
o All players/parents (returning and new to the club) will need to complete that form and also submit a current health insurance card.
o New-to-the club players ALSO will need to submit a copy of their birth certificate or passport.
o All players must submit the first half of the yearly dues via a check made out to FC Westchester.
